Concierge Companion (Part Time)

Paladin Life Care is expanding and looking for exceptional Concierge Companions to join our team. This is a part-time position on Monday through Friday from 8am to 5pm. Evening hours may be required for special events. Local travel will be required 100% of the time. The Concierge Companion meets a wide variety of needs for Paladin Life Care’s clients and promotes their best quality of life. Many of our clients experience loneliness as they age. Their social circles shrink, and it’s easy for them to begin to feel isolated. As a Concierge Companion, you will be responsible for engaging with our clients and providing enjoyable, compatible companionship, meaningful conversation, and emotional support. We provide customized services to meet their specific needs in-home and outside the home. You will work with clients who are of sound mind and capable of handling their activities of daily living on their own.

General Duties

  • Attend Paladin Life Care staff meetings for client updates and in-service and staff development as required
  • Provide transportation and accompany clients for errands, such as hair appointments, prescription pickup, and shopping
  • Provide transportation to appointments and bring clients into the facility for appointments
  • Accompany clients on a refreshing walk or for light gardening
  • Escort clients to the restaurant of their choice and provide conversation during the meal
  • Assist clients with their correspondence, hobbies, or crafts
  • Enjoy puzzles, play cards, board games, or other types of social games with clients
  • Attend an entertainment event, social or family event, or special function with clients
  • Perform other duties assigned by Management

Position Requirements

  • Minimum of five years of work experience
  • Bachelor’s Degree
  • Participate in continuing education opportunities
  • Strong written and verbal communication skills

Communication

  • Communicate effectively verbally and in writing within established timeframes
  • Use professional writing skills when completing case notes and when communicating with clients and other professionals
  • Enter and update information into Paladin Life Care’s secured data entry system and the client overview and calendars according to policy
  • Document client notes based on established criteria:
  • Enter notes within 48 hours
  • Ensure that notes are clear, concise, and comprehensive
  • Make sure that notes contain both objective and subjective information
  • Follow the established format for notes
  • Ensure that notes are grammatically correct with no spelling errors
  • Document reimbursements based on established procedures
  • Communicate issues/concerns up the chain of command
  • Report actual or potential cases of abuse/negligence to Paladin Life Care management, the client’s guardian, and all other authorities

Client Care Manager (Part Time)

Paladin Life Care is hiring a part-time Client Care Manager (CCM) to serve our elderly and disabled clients.  The CCM is a trusted advisor who will analyze a client’s situation and then research and create strategies within a Plan of Care that assist and contribute to a well-rounded quality of life These components include housing, finances, emotional stability, and living conditions.  The CCM serves as a liaison between the client and various service providers (i.e. in-home care companies, movers, organizers, utility companies, etc.).  The CCM works collaboratively with the RN Care Manager to ensure that resources are coordinated between medical and non-medical needs for each client.  Also, the Client Care Manager collaborates with the Daily Money Manager on behalf of their mutual client.  Your passion to make a positive difference in the lives of others is critical for your success in this role!  This is NOT home health care or skilled nursing services.

Nurse Care Managers (Part Time and Full Time)

Paladin Life Care is hiring part-time and full-time Nurse Care Managers (NCM).  The NCM is a trusted advisor who serves as a medical advocate on the client’s care team.  The person in this role is responsible for conducting assessments and developing a Plan of Care to support a well-rounded quality of life.  Areas of focus include:  identifying medical and functional health care needs; residential and living conditions; and emotional stability.  The NCM works collaboratively with the Client Care Manager to ensure that resources are available for the client to support his/her needs.  The passion to make a difference in the lives of others is the foundation for success in this role.

The NCM works with the client to find alternative treatment solutions that are satisfactory for him/her and the physician as needed.  Assists the client in locating new or alternative providers.  Conducts research to identify relevant standards and guidelines which support or offer an alternative to provider recommendations.

Position Requirements

  • Must hold a current RN or LPN license in VA, MD or DC in good standing
  • Minimum of 5 years of clinical experience
  • Medical, Surgical, Rehabilitation, or Psychiatry experience preferred
